Many seniors do not access needed services because of system mistrust, lack of knowledge about their options and how to navigate them, and fear of high costs. In particular, African American elders lag behind in social determinants of health as a result of the cumulative effects of racism.

Together, Vintage and Macedonia Family and Community Enrichment Center (FACE) will work in the Hill District, East Liberty, and Homewood, home to 11,423 seniors (2020 Census). As trusted community partners, we will engage elders and provide the support needed to close care gaps, improve social determinants of health, and empower them to age in place.

This human service partnership will work with Highmark Wholecare and UPMC Health Plan to demonstrate the value of these health outcomes, with the ultimate goal of sustainable insurance reimbursement. Beyond improving individual lives, we will work to create a new, replicable model of collaboration between healthcare and human services for seniors.
